· Visiting clients referred to the Careline Service to assess their need for the Service and identify and demonstrate the most suitable type of alarm and telecare equipment to meet their requirements.

• Collect and document personal data from the client in a sensitive manner and ensure that this complies with current data protection procedures regarding privacy and confidentiality.

• Carry out in-depth assessments as required to identify and refer clients to the Supporting People Services or to refer on to organisations such as Age Concern or the CAB to help them achieve their maximum income to enable them to pay for such things as the Careline service.

• Arrange, where required, any adaptations necessary for the safe installation of the equipment, e.g. electrical or telephone socket, key safes etc.

• Programme the equipment, install it in the customer’s home, demonstrate the use of the system and maintain records.

• Undertake a programme of follow-up visits to Careline clients (currently twice a year), and/or as needs arise, to ensure the equipment is in order, to check details and to signpost clients to other relevant services to meet their needs, enabling them to maintain their well-being and independence.

• Homelessness assessment done for homeless client and placements made

• Visiting Housing Association tenants in accordance with any contracts, and by arrangement.

• Visiting clients in response to urgent and non-urgent problems or issues with the Careline equipment.

• Organise and manage all the procedures to close a case and recover all equipment when a client no longer requires the service.

• Liaise with other colleagues internally and externally as appropriate to ensure continued appropriate use of the system and a joined-up approach to the support of the clients.

• Work with the Administrative Officer to liaise between the Exchequer Services team and the clients to monitor credit control issues to ensure payments are received for the Careline service.

• Work with the Administrative Officer to maintain up to date written and electronic records of all Careline activity and to ensure accurate statistics are available.

• Follow Careline procedures and protocols at all times in relation to such things as confidentiality, data protection, health and safety, client contact, handling of electrical equipment, and to feedback and actively contribute to the development of the service.

• Keep up to date with any advancement in technology, take part in appropriate training opportunities and attend staff meetings.

• Support the Careline Coordinator to undertake promotional events and demonstrations of equipment to groups of people and organisations as required.

• Actively contribute to the aims and objectives of the Community Partnerships Team and support colleagues as necessary.

• Carry out any other duties, which might reasonably be required.

Working hours: Shift work 7am-7pm and 7pm-7am Monday to Sunday, 12 hour shifts. Week 1, 3 days, week 2, 4 days week 3 3 days, week 4 4 days and week 5 off.

2 full time Social Workers to join the Discharge to Assess Team – currently it is not known how long the post will be for.

The role requires the SW to be able to travel around the city of Bristol to assess service users in care homes who have recently been discharged from hospital and require an assessment of need to identify any longer terms needs to enable them to return home or to if required identify longer term placements.

The service users are only funded in temporary nursing homes whilst they receive rehab or their Care Act assessment is completed for up to 28 days so there is a need to work in quite a time focussed way to facilitate discharges from these beds.

Rate - £28 per hour

Children’s

Full time Social Worker needed for the Through Care Team. Candidates must have experience within a Children in Care setting, with a minimum of 2-3 years’ experience.

Current team structure: 8 x Through Care consisting of 3 x SW, 2 x PA, 1 x Family Support, 1 x Unit co-Ordinator

Previous experience working with homeless children, understanding how to find suitable providers.

Rate - up to £31ph dependent on experience.
